تعیین تیپهای کاست کروموزومی ژن mecAدر ایزولههای استافیلوکوکوس اپیدرمیدیس جداشده از عفونتهای دستگاه ادراری در استان چهارمحال و بختیاری

چکیده

چکیده
مقدمه:از آنجایی که استافیلوکوکوس اپیدرمیدیس مهمترین گونه از استافیلوکوکهای کواگولاز منفی میباشد و عفونتهای مختلف بیمارستانی از جمله عفونتهای دستگاه ادراری را ایجاد میکند، توجه به الگوی مقاومتی این ارگانیسم به ویژه مقاومت به متیسیلین از اهمیت خاصی برخوردار میباشد. در این راستا، مطالعه حاضر با هدف تعیین الگوی مقاوت آنتی بیوتیکی سویه­های استافیلوکوکوس اپیدرمیدیس جداسازی شده از نمونه­های ادراری در بیمارستانهای سطح استان چهارمحال و بختیاری انجام شد.
مواد و روشها: تعداد 50 نمونه ادرار از افراد مبتلا به عفونتهای دستگاه ادراری (در حجم 20 میلیلیتر) در ظرف استریل نمونه­گیری ادرار از بیمارستانهای سطح استان چهارمحال و بختیاری گرفته شد و به روش کشت میکروبی و مولکولی آزمایش گردید. پس از تأیید مولکولی ایزوله­های جداشده از آزمایش انتشار دیسکی ساده جهت تعیین الگوی مقاومت آنتی بیوتیکی و از روش PCR (Polymerase Chain Reaction) به­ منظور تعیین تیپهای کاست کروموزومی ژن mecAاستفاده شد.
یافته­ها: از میان نمونه های مورد بررسی، 23 نمونه (46درصد) آلوده به استافیلوکوکوس اپیدرمیدیس بودند و بر مبنای آزمایشPCRواجد ژن 16srDNAشناخته شدند. الگوی مقاومت آنتی بیوتیکی این نمونه­ها به روش آنتی بیوگرام مطابق با معیار CLSI (Clinical & Laboratory Standards Institute) تعیین گردید. تمام ایزوله­های مورد مطالعه دارای مقاومت آنتی بیوتیکی چندگانه بودند که در این میان بیشترین مقاومت آنتی­بیوتیکی نسبت به دو آنتی­بیوتیک پنیسیلین ( 65/95درصد) و تتراسایکلین ( 30/91درصد) و کمترین میزان مقاومت نسبت به آنتی بیوتیک نیتروفورانتوئین (78/34درصد) بود. همچنین، 91/73 درصد از ایزوله­ها حامل ژن mecA بودند که تیپ SCCmecШبا فراوانی 05/47 درصد شایعترین تیپ کاست کروموزومی ژن mecAدر ایزوله­های مقاوم به متیسیلین بود.
نتیجه­گیری: شیوع مقاومت به طیف وسیعی از آنتی بیوتیکهای خطوط اول و دوم درمانی در میان سویه های استافیلوکوکوس اپیدرمیدیس نشاندهنده پراکندگی و انتشار این سویه­ها در بیمارستانهای استان چهارمحال و بختیاری می­باشد و لازم است که داروی مورد نیاز براساس نتیجه آنتی­بیوگرام انتخاب گردد
