Therefore, attention to the resistance pattern of this organism, especially resistance to methicilin, is of particular importance. The aim of this study was to determine the antibiotic resistance pattern of Staphylococcus epidermidis strains isolated from urine specimens in hospitals of Chaharmahal and Bakhtiari province, Iran. Materials and Methods: This study was conducted on 50 urine samples (in volumes of 20 ml) obtained from patients with urinary tract infections from ChaharMahal and Bakhtiari hospitals in sterile containers. The samples were tested by culture and molecular methods. After the molecular confirmation of the isolated strains, a simple disk diffusion test was used to determine the antibiotic resistance pattern. In addition, polymerase chain reaction (PCR) method was employed to determine the Staphylococcal cassette chromosome mec (SCCmec) types. Results: Out of the 50 samples, 23 specimens (46%) were infected with Staphylococcus epidermidis, which had 16srDNA gene in the PCR test. Antimicrobial resistance pattern was determined according to the Clinical and Laboratory Standards Institute. All isolates had multi-antibiotic resistance. In this regard, the isolates showed the highest resistance to penicillin (95.65%) and tetracycline (91.30%) and the lowest resistance to nitrofurantoin (34.78%). Furthermore, 73.91% of the isolates were the carriers of mecA gene. The SCCmec III (47.05%) was the most common type of chromosomal cassette mecA gene in methicillin-resistant isolates. Conclusion: The prevalence of resistance to a wide range of antibiotics in the first- and second-line treatment among strains of Staphylococcus epidermidis shows the distribution and dissemination of these strains in the hospitals of ChaharMahal and Bakhtiari province. Consequently, it is necessary to select the medication based on the antibiogram result.}, keywords = {Antibiotic resistance,SCCmec types,Staphylococcus epidermidis,Urinary tract infections}, title_fa = {تعیین تیپهای کاست کروموزومی ژن mecAدر ایزولههای استافیلوکوکوس اپیدرمیدیس جداشده از عفونتهای دستگاه ادراری در استان چهارمحال و بختیاری}, abstract_fa = {چکیده مقدمه:از آنجایی که استافیلوکوکوس اپیدرمیدیس مهمترین گونه از استافیلوکوکهای کواگولاز منفی میباشد و عفونتهای مختلف بیمارستانی از جمله عفونتهای دستگاه ادراری را ایجاد میکند، توجه به الگوی مقاومتی این ارگانیسم به ویژه مقاومت به متیسیلین از اهمیت خاصی برخوردار میباشد. در این راستا، مطالعه حاضر با هدف تعیین الگوی مقاوت آنتی بیوتیکی سویههای استافیلوکوکوس اپیدرمیدیس جداسازی شده از نمونههای ادراری در بیمارستانهای سطح استان چهارمحال و بختیاری انجام شد. مواد و روشها: تعداد 50 نمونه ادرار از افراد مبتلا به عفونتهای دستگاه ادراری (در حجم 20 میلیلیتر) در ظرف استریل نمونهگیری ادرار از بیمارستانهای سطح استان چهارمحال و بختیاری گرفته شد و به روش کشت میکروبی و مولکولی آزمایش گردید. پس از تأیید مولکولی ایزولههای جداشده از آزمایش انتشار دیسکی ساده جهت تعیین الگوی مقاومت آنتی بیوتیکی و از روش PCR (Polymerase Chain Reaction) به منظور تعیین تیپهای کاست کروموزومی ژن mecAاستفاده شد. یافتهها: از میان نمونه های مورد بررسی، 23 نمونه (46درصد) آلوده به استافیلوکوکوس اپیدرمیدیس بودند و بر مبنای آزمایشPCRواجد ژن 16srDNAشناخته شدند. الگوی مقاومت آنتی بیوتیکی این نمونهها به روش آنتی بیوگرام مطابق با معیار CLSI (Clinical & Laboratory Standards Institute) تعیین گردید. تمام ایزولههای مورد مطالعه دارای مقاومت آنتی بیوتیکی چندگانه بودند که در این میان بیشترین مقاومت آنتیبیوتیکی نسبت به دو آنتیبیوتیک پنیسیلین ( 65/95درصد) و تتراسایکلین ( 30/91درصد) و کمترین میزان مقاومت نسبت به آنتی بیوتیک نیتروفورانتوئین (78/34درصد) بود. همچنین، 91/73 درصد از ایزولهها حامل ژن mecA بودند که تیپ SCCmecШبا فراوانی 05/47 درصد شایعترین تیپ کاست کروموزومی ژن mecAدر ایزولههای مقاوم به متیسیلین بود. نتیجهگیری: شیوع مقاومت به طیف وسیعی از آنتی بیوتیکهای خطوط اول و دوم درمانی در میان سویه های استافیلوکوکوس اپیدرمیدیس نشاندهنده پراکندگی و انتشار این سویهها در بیمارستانهای استان چهارمحال و بختیاری میباشد و لازم است که داروی مورد نیاز براساس نتیجه آنتیبیوگرام انتخاب گردد}, keywords_fa = {استافیلوکوکوس اپیدرمیدیس,تیپهای,SCCmecعفونتهای دستگاه ادراری,مقاومت آنتی بیوتیکی}, url = {https://nnj.mums.ac.ir/article_11169.html}, eprint = {https://nnj.mums.ac.ir/article_11169_22e817d315a189848db0851e55b21b0c.pdf} }
