شناسایی و اولویت بندی شاخص های ارزیابی عملکرد بیمارستان با ترکیبی از کارت امتیازی متوازن و تکنیک دلفی (بیمارستان منتصریه)
مقدمه : بیمارستان ها برای بهبود بخشیدن به اهداف استراتژیک مندرج در نقشه استراتژی، نیاز دارند که آنها را با استفاده از شاخص ها اندازه گیری نمایند. هدف پژوهش شناسایی و اولویت بندی شاخص های ارزیابی عملکرد بیمارستان با استفاده از تکنیک دلفی و کارت امتیازی متوازن است. روش کار: این تحقیق از نوع پژوهش های کیفی بود که در بیمارستان منتصریه مشهد در سال 1394 انجام شد. جامعه ی آماری پژوهش 12 نفر از مدیران ارشد بیمارستان و اساتید خبره بهداشت و درمان بودند که به روش نمونه گیری فرد ماهر انتخاب شدند. ابتدا با بررسی متون و سپس تشکیل جلسات بحث گروهی، 68 شاخص در مناظر کارت امتیازی متوازن قرار داده شد. سپس با استفاده از تکنیک دلفی طی دو مرحله 34 شاخص نهایی گردید. روایی صوری و محتوایی شاخص ها با چند بار بازنگری با نظر تیم تحقیق و روایی سازه ی آن با دسته بندی شاخص ها محرز گردید. یافته ها: در این پژوهش مهمترین شاخص های ارزیابی عملکرد براساس برنامهریزی استراتژیک بیمارستان در قالب مناظر چهارگانه کارت امتیازی متوازن انتخاب شدند: 7 شاخص در منظر رشد و یادگیری، 13 شاخص در منظر فرایندها، 7 شاخص در منظر مشتری و 7 شاخص در منظر مالی. نتیجه گیری: با توجه به اینکه شاخص ها در سطوح بالای سازمان تعریف می شوند مدیران باید اصول علم اندازه گیری را درک کرده تا با انتخاب درست متغیرهای کنترلی ضمن استفاده موثر از شاخص ها، از هزینههای احتمالی جلوگیری کنند.

کارایی گروه درمانی هیجان محور بر تنظیم هیجان و دوره های پرخوری در زنان مبتلا به اختلال پرخوری
مقدمه: بدتنظیمی هیجان یکی از عوامل زمینه ساز و نگهدارنده پرخوری است. در این ارتباط، هدف از پژوهش حاضر تعیین اثربخشی گروه درمانی هیجان محور بر تنظیم هیجان و دورههای پرخوری در زنان مبتلا به اختلال پرخوری بود. مواد و روشها: پژوهش حاضر از نوع مطالعات نیمهتجربی با طرح پیش آزمون- پس آزمون با پیگیری شش هفته ای با گروه کنترل بود. جامعه آماری پژوهش را کلیه زنان مبتلا به اختلال پرخوری (Binge Eating Disorder)تهران که به مراکز درمان چاقی مراجعه کرده بودند تشکیل دادند. نمونه به روش در دسترس انتخاب شد و شامل 26نفر بود که در یکی از مراکز درمان چاقی اختلال پرخوری آنها تشخیص داده شده بود. افراد نمونه به صورت تصادفی در دو گروه هشت نفری کنترل و آزمایش قرار داده شدند و پرسشنامه دشواری در تنظیم هیجان و پرخوری (Difficulties in Emotion Regulation Scale)را تکمیل نمودند. گروه آزمایش طی 29جلسه (مدت هر جلسه یک ساعت و نیم بود) در مداخله شرکت کردند؛ اما گروه کنترل تنها پس از اتمام مطالعه و برای رعایت اخلاق مورد مداخله قرار گرفتند. در پایان دوره مداخله، پرسشنامه مجدداً توسط گروههای آزمودنی تکمیل گردید. داده ها با استفاده از آمار توصیفی میانگین و انحراف استاندارد و آزمون تحلیل واریانس اندازه گیری مکرر توسط نرم افزار SPSS 24 در سطح 0/05 تحلیل شدند. یافتهها: گروه درمانی مبتنی بر رویکرد درمان متمرکز بر هیجان در گروه آزمایش به طور معناداری (P≤0/05)منجر به کاهش نمره کلی دشواری هیجانی و شاخص دورههای پرخوری در اختلال پرخوری گردید. نتیجهگیری: براساس یافته ها میتوان گفت که درمان متمرکز بر هیجان باعث بهبود نشانه های اختلال پرخوری میشود

بررسی ویژگیهای ریخت شناسی زائده استیلوئید استخوان تمپورال
مقدمه:سندرم Eagleبا علائم درد شدید و محدودیت در حرکات گردن و دیسفاژی همراه میباشد. این اختلال به دلیل طویل و یا کلسیفیه شدن زائده استیلوئید و رباطهای اطراف آن و فشار بر باندل نورواسکولار اطراف آن پدید می آید. در این ارتباط، بررسی مورفومتری زائده استیلوئید برای آنتروپولوژیستها و آناتومیستها اهمیت دارد. با توجه به اهمیت دانستن مورفومتری زائده استیلوئید برای دندانپزشکان و آنتروپولوژیستها، در پژوهش حاضر به بررسی ویژگیهای مورفومتریک زائده استیلوئید در جمجمههای خشک پرداخته شده است. مواد وروشها:مطالعه توصیفی حاضر در سال 1396در ارتباط با 24جمجمه خشک که هیچ دفرمیتی و مشکل پاتولوژیکی نداشتند صورت گرفت. زائده استیلوئید بلندتر از 2/5سانتیمتر بهعنوان زائده استیلوئید بلند در نظر گرفته شد. زاویه بین محور فرانکفورت و محور طولی زائده به عنوان زاویه قدامی استیلوئید اندازهگیری گردید. دادهها وارد نرم افزار SPSS 20شدند و توسط آمار توصیفی شامل: فراوانی، درصد فراوانی و T-testآنالیز گردیدند. سطح معناداری نیز معادل P<0/05در نظر گرفته شد. یافتهها: میانگین طول زائده استیلوئید 1/36±0/86و میانگین ضخامت زائده استیلوئید 0/41±0/11سانتیمتر بود. زاویه بین محور فرانکفورت و محور طولی زائده استیلوئید نیز معادل 54/45±16/18بهدست آمد. همچنین زائده استیلوئید بلند در 8/3درصد از جمجمهها مشاهده شد. نتیجهگیری:در مطالعه حاضر اندازههای زائده استیلوئید به جمعیت آسیایی و به ویژه هند نزدیک بود. بررسی زائده استیلوئید میتواند برای پزشکان، دندانپزشکان، آناتومیستها و متخصصان پزشکی قانونی سودمند باشد. در این راستا، مطالعاتی با نمونههای بیشتر در مورد زائده استیلوئید استخوانهای جمجمه و نیز تصاویر رادیوگرافی پانورامیک و ام آر آی (MRI: Magnetic Resonance Imaging) میتواند اطلاعات کاملی را ارائه دهد.

تعیین ارتباط عملکرد جنسی با افسردگی و اضطراب در زنان یائسه مراجعه کننده به مراکز بهداشتی-درمانی شهرستان تربت حیدریه سال 1395
مقدمه : امروزه سلامت زنان پائسه مورد توجه و اهمیت قرار گرفته است. از آن جایی که عملکرد جنسی جز مهمی از زندگی زنان است, تغییر در آن که به خصوص و بیشتر در دوران یائسگی اتفاق می افتد, میتواند تاثیر زیادی بر سلامت ایشان بگذارد. همچنین یائسگی خود با تغییرات سلامتی متعددی مانند اضطراب ، افسردگی, تغییرات میل جنسی، خشکی واژن و. همراه است. این مطالعه با هدف تعیین ارتباط عملکرد جنسی با افسردگی و اضطراب در زنان یائسه مراجعه کننده به مراکز بهداشتی – درمانی شهرستان تربت حیدریه سال 1395 صورت گرفت . مواد و روش ها: این پژوهش یک مطالعه توصیفی – مقطعی بود که تعداد 200 نفر از زنان مراجعه کننده به مراکز بهداشتی شهرستان تربت حیدریه به صورت نمونه گیری در دسترس انتخاب شدند. ابزار گردآوری داده ها شامل پرسشنامه ی اطلاعات دموگرافیک، عملکرد جنسی (FSFI) و افسردگی و اضطراب زیگموند و اسنیت (HADS) بود. یافته ها: نتایج حاصل از پژوهش نشان داد که (72.5%) از زنان یائسه شرکت کننده در پژوهش عملکرد جنسی مطلوبی نداشتند. همچنین بین عملکرد جنسی و افسردگی و اضطراب ارتباط آماری معنی داری وجود داشت, به طوری که هر چه میزان افسردگی و اضطراب بیشتر شود, عملکرد جنسی نامطلوب تر است (0.05>P). نتیجه گیری: براساس نتایج ، فقط کمتر از نیمی از زنان عملکرد جنسی مطلوب داشته و افسردگی و اضطراب با اختلالات عملکرد جنسی رابطه ی مستقیم داشت, در نتیجه لزوم یک برنامه ریزی منسجم در جهت ارتقای کیفیت و سلامت زندگی زنان یائسه مورد نیاز است .

فراوانی ژنهای ebp A, ebp B و ebp Cدر ایزولههای انتروکوکوس فکالیس تولیدکننده بیوفیلم جدا شده از گوشت قرمز در شهرستان شهرکرد، سال 1396
مقدمه: انتروکوکوس بهعنوان یک باکتری مقاوم به بیشتر آنتیبیوتیکهای مصرفی قادر به انتشار ژنهای مقاومت به سایر گونهها ست این باکتری بهدلیل توانایی تشکیل بیوفیلم از قدرت بیماریزایی بالایی برخوردار است. تحقیق حاضر با هدف بررسی فراوانی انتروکوکوس فکالیس در گوشت گوساله عرضه شده به بازار مصرف شهرستان شهرکرد و بررسی ارتباط ژنهای ebp A, ebp B و ebp Cدر ایزولههای انتروکوکوس فکالیس تولیدکننده بیوفیلم جدا شده از گوشت قرمز صورت گرفت. مواد و روشها: در این مطالعه توصیفی-مقطعی 80 نمونه گوشت با روشهای بیوشیمیایی و مولکولی از نظر وجود انتروکوکوس فکالیس مورد بررسی قرار گرفتند. ردیابی ژنهای ebp A, ebp B و ebp C در حضور زوج پرایمرهای اختصاصی صورت گرفت. تجزیه و تحلیل آماری با استفاده از نرم افزار Prism7 و آزمون دقیق فیشر صورت گرقت. یافتهها: از 80 نمونه گوشت گوساله مورد بررسی در 31 ایزوله، انتروکوکوس فکالیس تشخیص داده شد. در روش میکروتیتر پلیت بهمنظور تشکیل بیوفیلم، در 25 ایزوله ( 80/64%) بیوفیلم تولید گردید. ژن ebp A در 28 ایزوله (90/32%)، ژن ebp B در 10 ایزوله (32/25%) و ژن ebp C در هیچ کدام از ایزولهها (0%) گزارش نگردید. نتیجه گیری: بیماریهای منتقله توسط غذا از مشکلات شایع در بخش سلامت و بهداشت عمومی هستند. در این میان باکتریها از جمله مهمترین عوامل ایجادکننده عفونتها و مسمومیتهای غذایی محسوب میشوند برای جلوگیری از آلودگی میکروبی مواد غذایی، آموزش افراد، رعایت اصول بهداشتی و نظارت در آماده سازی، حمل و نقل، ذخیره سازی و عرضه مواد غذایی ضروری می باشد.

مقدمه و هدف: عفونت های دستگاه ادراری یکی از شایع ترین عفونت های باکتریایی است که عمدتا توسط اشریشیاکلی اوروپاتوژنیک (UPEC) ایجاد می گردد. هدف از این مطالعه تعیین شیوع انواع سروگروپ های O در باکتری های اشریشیاکلی جدا شده از بیماران مبتلا به عفونت ادراری در استان اصفهان و دستهبندی ژنتیکی ایزولههای سروگروپ O25، به روش RAPD-PCR می باشد. روش کار: 226 نمونه ادرار بیماران مبتلا به عفونت ادراری از بیمارستان های استان اصفهان جمع آوری و مورد بررسی قرار گرفتند. جدایه های اشریشیاکلی با استفاده از روش های بیوشیمیایی شناسایی شدند. سروگروپ های این جدایه ها به روش PCR تعیین و دسته بندی ژنتیکی ایزوله های دارای سروگروپ O25 با استفاده از روش RAPD-PCR انجام گرفت . یافته ها: از میان کل نمونه های مورد مطالعه، 96 جدایه اشریشیاکلی جدا گردید. شایع ترین انواع آنتی ژن O، O25 (5/37 درصد)، O21 (37/9 درصد) و O6 (33/8 درصد) بودند دسته بندی ژنتیکی ایزوله های دارای سروگروپ O25، 27 پروفایل مختلف را در میان این 36 جدایه با ضریب تشابه بالای 80 درصد نشان داد. نتیجه گیری: در اشریشیاکلی های مولد عفونت ادراری، سروگروپ های O می توانند با الگوی عوامل ویرولانس در هر سویه در ارتباط باشند. روش RAPD-PCR می تواند برای تشخیص سویه های اشریشیاکلی از نمونه های بالینی و یا محیطی مورد استفاده قرار گیرد. تنوع ژنتیکی بالا در جدایه ها نشانگر منابع مختلف آلودگی دستگاه ادراری به اشریشیاکلی می باشد.

مروری بر مداخلات آموزشی پیشگیرانه از اچ آی وی/ ایدز در ایران
زمینه و هدف: سازمان جهانی بهداشت اساسی ترین راهکارهای مقابله با مشکلات رفتاری را ارتقای سطح آگاهی های عمومی در خصوص ماهیت رفتارهای پرخطر و مشکلات ناشی از آن، راههای انتقال و پیشگیری از آن می داند. هدف از مطالعه مروری حاضر بررسی مداخلات آموزشی پیشگیرانه از اچ آی وی/ ایدز در ایران می باشد. روش بررسی: در این مطالعه مرور سیستماتیک مقالات فارسی و انگلیسی مناسب و مرتبط با ﺟﺴــﺘﺠﻮ در ﭘﺎﯾﮕــﺎه ﻫــﺎی اﻃﻼﻋــﺎﺗﯽ Google scholar،Scopus ،Cochrane،PubMed و ISIو پایگاه های فارسی SID و Magiran با استفاده از ﮐﻠﯿــﺪ واژه ﻫــﺎی فارسی شامل آموزش، آموزش بهداشت، مداخله، ایدز/ اچ آی وی، آگاهی، نگرش، تئوری رفتاری و معادل انگلیسی از سال 1395-1382 گردآوری شدند. در اﻧﺘﻬﺎی ﺟﺴــﺘﺠﻮ 130 مقاله بدست آمد. ﭘﺲ از حذف تکرار و ﻏﺮﺑﺎﻟﮕﺮی ﻋﻨﻮان، ﭼﮑﯿﺪه و متن اصلی 95 ﻣﻘﺎﻟﻪ ﺧﺎرج و 35 ﻣﻘﺎﻟﻪ وارد مطالعه شدند. یافتهها: در اﯾﻦ ﻣﻄﺎﻟﻌﻪ ﻣﻘﺎﻻت ﻣﻮرد ﺑﺮرﺳﯽ ﺑﻪ دو دﺳﺘﻪ ﻋﻤﺪه ﺗﻘﺴﯿﻢ ﺷﺪﻧﺪ: 1- ﻣﻄﺎﻟﻌﺎﺗﯽ ﮐﻪ ﺗﺌﻮری ﻫﺎ و ﻣﺪل ﻫﺎی آﻣﻮزش ﺑﻬﺪاﺷﺖ ﻣﺒﻨﺎی ﻣﺪاﺧﻠﻪ آﻣﻮزﺷﯽ در آن ﻫﺎ ﺑﻮد که تعداد آنها 15 مطالعه (42/9 درصد)2- مطالعاتی که ﻣﺪاﺧﻼت آﻣﻮزﺷﯽ آن ﻫﺎ ﺑﺪون اﺳﺘﻔﺎده از ﺗﺌﻮری ﻫﺎ و ﻣﺪل ﻫﺎی آﻣﻮزش ﺑﻬﺪاﺷﺖ اﻧﺠﺎم شده است که تعداد آنها 20 مطالعه (57/1 درصد)بود. نتیجهگیری: ﻣﺪاﺧﻼت ارﺗﻘﺎ ﺳﻼﻣﺖ در مطالعات انجام شده بر فاکتورهای مورد بررسی تاثیر مثبت داشته است. توصیه می گردد که در طراحی مداخلات آموزشی مفاهیم استیگما نسبت به بیماران جهت تاثیرگذاری و بهبود نگرش و باورهای رفتاری افراد جامعه نسبت به اچ آی وی/ایدز گنجانده شود.
