تعیین باقی مانده آنتی بیوتیک سولفانامید در شیرخام، گوشت وتخم مرغ به روش الایزا
مقدمه: امروزه وجود باقیماندههای آنتیبیوتیکی یکی از دغدغههای اصلی صنعت مواد غذایی به شمار میرود. از جمله آثار منفی استفاده نابهجا از آنتیبیوتیکها میتوان به اختلالات متابولیکی، واکنشهای سمی و تحریکی و حساسیت و آلرژی اشاره کرد. مواد و روشها: در این مطالعه توصیفی تعداد 540 نمونه از مواد غذایی شامل: شیر خام گاو (80=n)، گوشت گاو (50=n)، گوشت گوسفند (50=n)، کبد گاو (50=n)، کلیه گاو (50=n)، کبد گوسفند (50=n)، کلیه گوسفند (50=n)، گوشت مرغ (80=n) و تخممرغ (80=n) بهطور تصادفی از مراکز جمعآوری شیر، کشتارگاهها و فروشگاههای موجود در شهر اصفهان طی سالهای 93-1392 جمعآوری شدند و میزان بقایای آنتیبیوتیک سولفانامید در آنها با استفاده از روش ELISA مورد آزمایش قرار گرفت. یافتهها: نتایج نشان داد که از 540 نمونه مورد بررسی، 58 نمونه (48/10 درصد) حاوی آنتیبیوتیک سولفانامید بودند. به این صورت که گوشت مرغ، شیر خام گاو، کبد گاو، کلیه گاو، گوشت گوسفند، تخممرغ، کبد گوسفند و کلیه گوسفند بهترتیب حاوی 2،5،4،25،20،12،12،6 درصدآنتیبیوتیک سولفانامید بودند. غلظت آنتیبیوتیک سولفانامید در نمونههای آلوده معادل 24/172-34/7 نانوگرم بر گرم بهدست آمد. علاوهبراین، میزان باقیمانده آنتیبیوتیک سولفانامید در گوشت مرغ بهطور معناداری بیشتر از سایر فرآوردههای مورد مطالعه بود (05/0P<). نتیجهگیری: با توجه به یافتهها، نظارت گسترده و دقیق بر تولید و عرضه فرآوردههای غذایی با منشأ دامی از جمله گوشت، ﺷﯿﺮ ﺧﺎم و تخممرغ و ارزیابی وجود باقیماندههای دارویی در آنها ضروری میباشد.

سنجش میزان تولید آنزیم پروتئیناز در گونههای کاندیدای جداشده از کاندیدیازیس دهانی بیماران سرطانی دریافتکننده شیمیتراپی بیمارستانهای تنکابن و رامسر
مقدمه:کاندیدیازیس یک عفونت قارچی شایع است که توسط گونههای مختلف کاندیدا ایجاد میشود. تولید آنزیمهای خارج سلولی نظیر پروتئیناز بهعنوان فاکتور بیماریزا نقش بهسزایی در چسبندگی و نفوذ مخمر به میزبان مستعد دارد. در این راستا، هدف از پژوهش حاضر بررسی میزان تولید آنزیم پروتئیناز در بیماران سرطانی مبتلا به کاندیدیازیسمی دهانی میباشد. مواد و روشها:تعداد 20 نمونه سواپ دهانی از بیماران سرطانی و غیرسرطانی مبتلا به کاندیدیازیس مراجعهکننده به مرکز انکولوژی بیمارستان رامسر و تنکابن در سال 1394-1393 جمعآوری گردید. این نمونهها در محیط سابورو دکستروز آگار حاوی آنتیبیوتیک کلرامفنیکل کشت داده شدند و ایزولههای مخمری با استفاده از روش فنوتایپیک مورد شناسایی قرار گرفتند. در ادامه برای ارزیابی فعالیت آنزیمی پروتئینازی از محیط اختصاصی استفاده گردید. قطر هاله تشکیلشده در اطراف هر کلونی نیز بهمنظور تولید آنزیم اندازهگیری شد. یافتهها:از 20 نمونه جمعآوریشده، از گروه بیماران سرطانی مبتلا به کاندیدیازیس دهانی 19 ایزوله مخمری کاندیدا (95 درصد) و از گروه شاهد شش ایزوله (30 درصد) جدا گردید. فراوانی کاندیدا آلبیکنس و کاندیدا گلابراتا بهترتیب معادل 16 (84 درصد) و 3 (16 درصد) بود. در افراد غیرسرطانی تمام ایزوله مخمری جداشده بهعنوان کاندیدا آلبیکنس شناسایی شد. میانگین قطر هاله در اطراف سلولهای مخمری تولیدکننده آنزیم پروتئیناز در افراد سرطانی مبتلا به کاندیدیازیس دهانی نسبت به گروه غیرسرطانی معادل 21/2 میلیمتر بهدست آمد. نتیجهگیری:تولید آنزیم پروتئیناز بهعنوان یک فاکتور ویرولانس مهم در ایجاد بیماری کاندیدیازیس دهانی در افراد سرطانی به مراتب بیشتر از افراد غیرسرطانی بود.

بررسی بیوانفورماتیک ژن های خانه دار در افتراق مایکوباکتریوم های سریع الرشد
مقدمه:مایکوباکتریومهای سریعالرشد (RGM: Rapid Growing Mycobacterium) قادر هستند عفونتهای ریوی، لنفاوی، پوست و بافت نرم را در انسان به وجود آورند. ما میتوانیم با استفاده از ژنهای خانهدار، این گروه از باکتریها را شناسایی کنیم. در این ارتباط، هدف از پژوهش حاضر مقایسه و ارزیابی سه ژن خانهدار در شناسایی و طبقهبندی برخی از مایکوباکتریومهای سریعالرشد با استفاده از درخت فیلوژنیک بود. مواد و روشها:برای انجام این مطالعه ابتدا توالیهای سه ژن 16SrRNA، hsp65و rpoBبرای 10 گونه سریعالرشد مایکوباکتریومی از پایگاههای NCBI (National Center for BiotechnologyInformation)، RDP (RemoteDesktopProtocol) و EMBL (EuropeanMolecularBiologyLaboratory) دریافت شد. سپس این توالیها توسط نرمافزار CustalWهمطراز گردیدند و درخت فیلوژنیک با استفاده از برنامه MEGA5ترسیم گشت. یافتهها:بر مبنای نتایج، الگوی شناسایی و افتراق هر ژن متفاوت بود. ژن 16SrRNAقادر به شناسایی و افتراق مایکوباکتریومهای سریعالرشد بیماریزایی چون مایکوباکتریوم آبسسوس (Abscessus) و چلونه (Chelonei) نبود. بهطور کلی، ژن rpoBنسبت به سایر ژنها بهتر عمل کرد؛ اگرچه قادر به شناسایی مایکوباکتریوم نووکاسترنس (novocastrense) نبود. نتیجهگیری:بهمنظور شناسایی کامل و صحیح مایکوباکتریومهای سریعالرشد باید هر سه ژن 16SrRNA، rpoBو hsp65بهطور همزمان مورد مطالعه قرار گیرند.

شیوع آسیبهای ناشی از وسایل نوک تیز و برنده آلوده و عوامل موثر بر آن
مقدمه:آسیب ناشی از وسایل تیز و برنده یکی از مهمترین خطرات بیولوژیک برای کارکنان بهداشتی- درمانی به شمار میآید و پیامد بیولوژیکی این آسیبها میتواند بسیار خطرناک بوده و زمینهساز انتقال بیماری باشد؛ از این رو هدف از پژوهش حاضر تعیین شیوع، علل و عملکرد پرسنل درمانیمرکز آموزشی- درمانی بیمارستان پنج آذر گرگان در برابر آسیبهای ناشی از وسایل نوکتیز و برنده آلوده میباشد. مواد و روشها:این مطالعه توصیفی- تحلیلی در سال 1394 در بیمارستان آموزشی- درمانی پنج آذر گرگان انجام شد. 345 نفر از پرسنل درمانی بخشهای اتاق عمل، بستری زنان و مردان، آزمایشگاه و اورژانس بهصورت سرشماری وارد مطالعه شدند. دادههای مربوط به شیوع و علل آسیب ناشی از وسایل نوکتیز و برنده و اقدامات پس از آن از طریق چکلیست جمعآوری گردیدند و یافتهها بهصورت صدک و درصد گزارش شده و با استفاده از آزمون Pearson- کای اسکوئر و سطح معناداری کمتر از 05/0 در نرمافزار SPSS 16مورد تحلیل قرار گرفتند. یافتهها:از بین 345 نفر از کارکنان درمانی که وارد مطالعه شدند، 246 نفر (5/69 درصد) زن بودند و میانگین سنی و انحراف معیار افراد مورد مطالعه معادل 8/7±9/30 سال بهدست آمد. همچنین، شیوع نیدلاستیک در بین کارکنان درمانی این مرکز معادل 3/81 درصد بود و مهمترین علل آسیبزایی، درپوشگذاری مجدد هنگام تزریقات گزارش شد. از سوی دیگر، دو اقدام مهم که توسط فرد پس از آسیب انجام شده بود، عبارت بود از: شستشو با مواد ضدعفونیکننده (3/20 درصد) و فشاردادن محل آسیب (1/16 درصد). شایان ذکر است که نتایج آزمون Pearson- کای اسکوئر، رابطه معناداری را بین جنسیت و آسیب با وسایل نوکتیز و برنده نشان داد (016/0P=). نتیجهگیری:شیوع نیدلاستیک در این مرکز آموزشی- درمانی نسبت به مطالعات مشابه در ایران و آمارهای جهانی بالاتر بوده و بیشترین شیوع در آزمایشگاه و آسیبزاترین موقعیت، درپوشگذاری مجدد بر سرنگ میباشد؛ بنابراین نیاز به آموزش مهارتهای شغلی برای پیشگیری از آسیب و نحوه مواجه با این مشکل برای کادر درمانی ضروری به نظر میرسد.

بررسی نقش بی اشتیاقی اخلاقی و خودنهان سازی در پیش بینی انگیزش درمان افراد وابسته به مواد مخدر
مقدمه:انگیزش گامی اساسی در پیشبرد اهداف درمان در اختلالات مرتبط با مواد است. در این ارتباط، پژوهش حاضر با هدف تعیین نقش بی اشتیاقی اخلاقی (Moral Disengagement)و خودنهان سازی (Self-Concealment) در پیشبینی انگیزش درمان افراد وابسته به مواد انجام شد. مواد و روشها: در این مطالعه همبستگی 120 نفر از جامعه افراد دارای وابستگی به مواد مخدر مراجعه کننده به مراکز ترک اعتیاد سرپایی شهر اردبیل در نیمه اول سال 1394 به روش نمونه گیری تصادفی خوشهای انتخاب شدند و به پرسشنامههای بیاشتیاقی اخلاقی Banduraو همکاران، خودنهان سازی Larsonو همکاران و انگیزش درمان Miller و همکاران پاسخ دادند. داده های جمعآوری شده با استفاده از آزمو های همبستگی Pearsonو تحلیل رگرسیون چندگانه تحلیل شدند. یافتهها:نتایج حاکی از آن بودند که رابطه بازشناسی با بیاشتیاقی اخلاقی و خودنهانسازی منفی میباشد (001/0>p). بین دوسوگرایی با نمره کلی بیاشتیاقی اخلاقی و مؤلفه های توجیه اخلاقی، مقایسه سودمند، پخش مسئولیت، تحریف پیامدها و خودنهان سازی نیز رابطهای منفی وجود داشت (05/0> (P علاوه براین، گامبرداری به طور منفی با نمره کل بی اشتیاقی اخلاقی و مؤلفه های توجیه اخلاقی، مقایسه سودمند، جا به جایی مسئولیت، پخش مسئولیت، اسناددادن سرزنش و خودنهان سازی مرتبط بود (05/0> (P. نتایج تحلیل رگرسیون نیز نشان دادند که 42 درصد از واریانس بازشناسی، 19 درصد از واریانس دوسوگرایی و 34 درصد از واریانس گامبرداری به وسیله بیاشتیاقی اخلاقی و خودنهان سازی تبیین میشود. نتیجهگیری:نتایج نشان دادند که بیاشتیاقی اخلاقی و خودنهان سازی میتوانند به صورت معناداری انگیزش درمان افراد وابسته به مواد را پیش بینی کنند. نتایج این مطالعه لزوم ارائه برنامه های آموزشی مقابله با بی اشتیاقی اخلاقی و خودنهان سازی جهت افزایش انگیزش درمان افراد وابسته به مواد را پیشنهاد میکند.
