Tramadol side effects are seizure and rhabdomyolysis. In this regard, the present report introduced a girl suffering from acute kidney injury and creatinine increase as a result of tramadol poisoning. Case Report: A girl was admitted to an intensive care unit with a diagnosis of tramadol overdose. Her symptoms included loss of conciseness and respiratory depression. She was intubated. Her urine toxicology test was positive for tramadol, and urea and creatinine levels were reported as 126 and 9.5 mg/dl, respectively. She underwent daily dialysis for 2 h; however, no decrease was observed in urea and creatinine levels. Daily dialysis increased to 3.5 h, and after 11 days, her creatinine and urea levels decreased to 3 and 20 mg/dl, respectively. The patient did not have any history of neurological diseases, and her brain computed tomography scan showed leukodystrophy. On the 12th day of hospitalization, the patient was separated from the ventilator and transferred to the ward. She was discharged after a week and recommended to continued psychiatric treatments. Conclusion: Tramadol belongs to opioids and is usually sold without prescription. Seizure, rhabdomyolysis, kidney failure, and brain complications due to long hypoxia are the reported side effects, which are increasing in recent years. Therefore, higher control of tramadol legal and illegal distribution is required.}, keywords = {Acute kidney injury,Cerebral hypoxia,hemodialysis,rhabdomyolysis,Tramadol,Seizure}, title_fa = {افزایش کراتینین در دختر جوان به دلیل مسمومیت با ترامادول}, abstract_fa = {مقدمه:ترامادول داروی شبه‌مخدری است که دو مکانیسم اثر دارد: اثر بر گیرنده‌های مو و مهار بازجذب نوراپی‌نفرین و سروتونین. از عوارض شایع مسمومیت با ترامادول می‌توان به تشنج و رابدومیولیز اشاره کرد. در این ارتباط، در پژوهش حاضر دختر جوانی معرفی می‌شود که به دنبال مسمومیت با ترامادول دچار افزایش کراتینین و نارسایی حاد کلیه شده است. معرفی بیمار: دختر جوان 17 ساله‌ای به دنبال مصرف تعدادی قرص ترامادول با اختلال هوشیاری و نارسایی تنفسی در بخش مراقبت‌های ویژه بستری شد و تحت تهویه مکانیکی قرار گرفت. آزمون سم‌شناسی ادرار از نظر ترامادول مثبت بود و میزان اوره معادل 126 و برای کراتینین معادل 5/9گزارش گردید. این بیمار روزانه به مدت دو ساعت تحت دیالیز قرار گرفت؛ اما کاهشی در میزان اوره و کراتینین مشاهده نشد. دیالیز روزانه به مدت 5/3 ساعت افزایش یافت و به‌تدریج اوره و کراتینین کاهش پیدا کرد و پس از 11 روز کراتینین به 3 و اوره به 30 رسید. شایان ذکر است که تغییراتی در سی‌تی‌اسکن مغز مشاهده شد که به‌تدریج بهبود یافت. روز دوازدهم بستری، بیمار از ونتیلاتور جدا شد و به بخش منتقل گردید و پس از یک هفته با حال عمومی خوب و با توصیه به انجام اقدامات بازتوانی مرخص شد. نتیجه‌گیری:ترامادول دارویی از گروه اوپیوئیدها است که بدون تجویز فروخته می‌شود. تشنج، رابدومیولیز، نارسایی کلیه و عوارض مغزی ناشی از هیپوکسی طولانی مواردی هستند که طی سال‌های اخیر تعداد آن‌ها افزایش یافته است؛ از این رو نظارت دقیق‌تر بر توزیع این دارو الزامی می‌باشد.}, keywords_fa = {تشنج,ترامادول,نارسایی کلیه,همودیالیز,هیپوکسی مغزی}, url = {https://nnj.mums.ac.ir/article_13908.html}, eprint = {https://nnj.mums.ac.ir/article_13908_1c873fd2ac6c1338c3a83814b82cc618.pdf} }
